docs: update all references from @v3 to @v4#43

Update version references across all READMEs and the generate-workflow skill to point to v4 after the zad-cli migration. Also update the API section in the root README to reflect that actions now use zad-cli instead of direct API calls.
Bugfix release only (config init .env handling). No API changes.
